The Caregiving Hassles Scale (CHS) consists of a questionnaire completed by caregivers of patients with dementia. Caregivers rate various patient requirements and behaviours on a 5-point scale of frequency, ranging from “Did not occur” to “A lot.”
This app allows the inventory to be completed electronically and the results uploaded to an appropriately configured server. Designated care providers can view the data to help design appropriate interventions for each patient.
